Current Plymouth City Council apprentices wishing to apply for this role must contact the recruiting manager to ensure that they are able to support them to satisfactorily complete their NVQ qualification. Apprentices must also have 6 months continuous service and satisfactorily completed their probationary period before applying.

Are you a highly motivated, organised person with excellent interpersonal skills? An exciting opportunity has now become available for one suitably experienced Community Outreach Worker to join our friendly team.

The Community Outreach team provide time limited support to a range of people across the city that have been assessed as requiring practical assistance with:
• Assisting Livewell Southwest with the review care packages
• Crisis Intervention & Supporting transitions
• Supporting social workers to gain a better understanding of a person's needs.
• Using assistive technology to better understand a person's needs whilst at home.
• Support to complete applications with an individual such as Devon Home Choice PIP applications and Grants.
• Support individuals with urgent housing needs (presenting as being homeless) and support individuals into alternative temporary accommodation as part of an emergency response.
• Provide an Appropriate Adult at Charles Cross Police station in response to individuals who are deemed to be suffering from mental ill health or other mental vulnerability (e.g. learning disability) that need support when in an police interview. disability) that need support when in an police interview.
• Obtain food parcels for individuals identified as being in financial crisis.
• Support to prepare an individual's home for hospital discharge.
• Support people at risk of self-neglect, aiming to prevent increased stay within hospital environment.
• Reduce the need for on-going adult social care input by working through specific routines and strategies for maintaining a safe home environment.

As a Community Outreach Worker you will be required to support the established care management process by working to support adults with a variety of needs identified outcomes in order to promote independence, health and wellbeing.

As a worker you will be expected to signpost, facilitate and involve those you support to access a range of services and agencies within the local/wider community.

The post holder will have knowledge of Care Management and the principles of Community Care and other relevant legislation NVQ3 in Care / Promoting Independence, or equivalent qualification.
