This is a real opportunity for people who want to work as part of a wider team to support those who have a variety of needs that require direct support as well as working with people to maintain their independence at home and contribute to their wellbeing.

We are looking for people to work within a range of service areas to undertake a variety of tasks supporting adults with a range of different needs. The areas include:-

The Vine Day Service This service provides a variety of centre and community based opportunities on a daily basis for up to 60 adults with learning disabilities and a range of complex health and social care support needs.

We are passionate about making a difference to the lives of adults with learning disabilities and complex needs and we offer:
- Independent Living Skills
- Work opportunities
- Community Interaction
- Swimming
- Dance / Drama groups
- Cycling / Adapted cycles
- Computer / Gadget skills using the latest technology
- Art / Craft / Pottery
- Textiles / Dress making

Independence at Home Plymouth City Council’s Independence@Home Team are looking for a number of caring, sensitive and enthusiastic individuals to join our established professional care team. Why not visit this link to find out more about what we do.

This is a real opportunity for people who want to work as part of a wider team to support those who have a variety of needs to retain their independence at home and contribute to their wellbeing.

You don’t need to have already worked in care, as full training and support is provided but you will already have some really important skills and understanding that you will be able to bring to this rewarding role

Community Outreach Team This service provides time limited support to people identified across the city as needing hands on advice and guidance relating to:
- Housing
- Managing debts, budgets and finances
- Support with form filling
- Support to access the community
- Independent travel
- Introduction to a range of local community based activities
- Support for introduction to specific organisations that can assist the person longer term Working across the city, the worker will have responsibility to plan their own working day and draw up a plan on how best they can support the person they are allocated to support.

You don’t need to have already worked in care, as full training and support is provided but you will already have some really important skills and understanding that you will be able to bring to these rewarding roles

These include:
· Effective communication skills, verbal and written to build rapport with individuals and record and report on aspects of their health and well-being.
· Good time management and organisational skills.
· A reasonable understanding of the need for dignity, respect and sensitivity when working with vulnerable and older people
· The ability to motivate and work with service users with a range of behaviours that at times may be challenging to manage

There are some physical requirements, for example being able to carry out personal care tasks with respect and dignity, and full training including, manual handling training, is provided.

It is essential that you have the following:-
· A good standard of literacy and numeracy skills.
· Ability to work towards the Health and Social Care Diploma Level 2
· A commitment to continuous professional development in accordance with National Care Quality Standards and Plymouth City Council’s training plan. (Depending on position)
